Incident Report

A formal disciplinary hearing will be held only after all other attempts of addressing the issue through the proper channels have not produced a satisfactory outcome or resolution.  For any required disciplinary hearing, the committee will be made up of a minimum of three (3) members of the FMBA Executive.  Any member of the FMBA executive committee that has either a real or perceived conflict of interest in the matter will be excluded from participating in the hearing.
